One piece of 18.0 inch by 12.0 inch paper folded in half, 9.0 inch by 12.0 inch page inserted. Front page, rectangular illustration of "ICE WORM WIGGLE" cartoon ice worms, Eskimos, black and white. Top header in red ink "AKH-TV-WU-YE-KEH….CHEECHAKOS" sides of rectangular illustration, then parallel black lines, red stars placed on both end, one red star on outside of lines in center, bottom / footer. Black ink "Words and Music by CAROL BEERY DAVIS" in left lower corner. "Featured by Marie Lysing Johnson" right lower corner. three diagonally placed red stars flow between inside, sheet music, bars and notes in black, words in red. Music printed on three full pages. Back page "ICE WORM WIGGLE DANCE" instruction on performing dance, 1.0 inch thick margins around text.
